Why a rotation sometimes does not stick
This is the one thing people actually search for on this subject. Turning a page in a viewer and turning it in the file are different operations, and only one of them survives being sent to somebody else.

What rotation actually changes
Every page in a PDF carries a rotation value, and the specification only allows 0, 90, 180 or 270 degrees. Saving a rotation updates that number and nothing else.
No pixels move and no content is redrawn, which is why the file size barely changes and why text stays selectable afterwards. It is one of the few edits you can make to a PDF with no quality cost at all.
It also means arbitrary angles are impossible. A page scanned three degrees crooked cannot be straightened here, because the format has nowhere to store that. Fixing genuine skew means re-scanning or editing the page as an image.
Why scanners get it wrong so often
A sheet feeder has no idea which way up the text is. It records the orientation the paper physically travelled through it, and if the stack went in rotated, every page inherits that.
Duplex scanning adds its own twist. Flipping along the long edge instead of the short one leaves every alternate page upside down, which is why documents sometimes need the whole file rotated and then half the pages turned again.
Rotating one page instead of all of them
Mixed orientations in a single document are normal and perfectly valid. A landscape chart or a wide table sitting inside a portrait report is the usual case.

- Rotate everything first, then fix the exceptions. Most documents are wrong in the same direction throughout.
